What Nobody Tells You About Sleep After 40
The reason you're waking up at 3am and lying there isn't stress. Here's what's actually happening, and what to do about it.

I wish more people talked about the shifts that happen when you move from your 30s into your 40s. Not just the hormonal changes everyone vaguely references, but the specific, under-the-hood stuff that starts affecting how you feel, how you recover, and yes, how you sleep.
Everyone knows sleep is important. That's not the problem. The problem is that most people can't seem to fix it no matter what they try. They've cut off screens an hour before bed. They've switched to decaf after noon. They've tried melatonin, magnesium, chamomile tea, white noise, blackout curtains, even prescription sleep aids. And they're still waking up at 3am, still dragging through their mornings, still wondering what they're missing.
What's rarely addressed are the actual physiological and biological shifts driving the problem. Things like cortisol dysregulation, low blood sugar overnight, declining sex hormones, and changes to deep sleep architecture that start happening in your late 30s and accelerate into your 40s. For women that means dropping progesterone alongside elevated tissue estrogen. I have become really intrigued by DrRay Peat's work on estrogen, stress and sleep. He flips the script on what the research shows and he talks a lot about how estrogen behaves as a stress hormone, so when progesterone falls and tissue estrogen goes unopposed, you're left with a hormonal environment that actively works against sleep. For men it means declining testosterone, and declining progesterone too. Most people don't realize men produce progesterone as well. It's not a female-only hormone, neither is estrogen. In men, progesterone serves as a precursor to testosterone and plays its own role in nervous system regulation and sleep quality. When it drops, sleep suffers for both sexes. These aren't lifestyle problems you can out-habit. They're biological, and they need a biological solution.
Let me walk you through what's really going on.
THE SLEEP PROBLEM NOBODY'S TALKING ABOUT
Here's what most sleep content gets wrong: it treats all sleep the same. Eight hours is eight hours, right? Nope. The hours matter far less than what's happening during those hours. The magic happens in deep sleep, specifically slow-wave sleep, and that's where most people are getting robbed.
During deep sleep your body releases human growth hormone, your brain flushes out metabolic waste through the glymphatic system, your cortisol resets, your muscles repair, and your metabolism gets recalibrated for the next day. Starting in our late 30s and into our 40s, the architecture of our sleep shifts. We naturally spend less time in those deep restorative stages. For women, the hormonal picture becomes particularly disruptive during this window. Progesterone, your primary calming, sleep-supporting hormone, drops significantly. At the same time, tissue estrogen tends to remain elevated or even rise. This matters because in Ray Peat's framework, estrogen functions as a stress hormone. It promotes excitation, raises cortisol, and competes directly with progesterone's calming effects. When progesterone falls and tissue estrogen goes unopposed, the nervous system becomes harder to down regulate at night. You're essentially sleeping with an elevated stress signal running in the background. Add increased cortisol sensitivity and temperature fluctuations on top of that, and you have a formula for broken sleep that no sleep hygiene checklist is going to fix. For men, the story is a little different. Testosterone declines steadily from the mid-30s onward, and lower testosterone is directly associated with reduced deep sleep, more nighttime waking, and worse sleep efficiency overall, according to research in the Journal of Clinical Endocrinology and Metabolism. Progesterone also falls in men across this same window, further reducing one of the body's natural nervous system calming signals. So regardless of your sex, the hormonal landscape of midlife is quietly working against your sleep, and most people have no idea that's even happening.

THE 3AM PROBLEM:
This is my favorite part to explain because it's one of those things that makes total sense once you hear it, and you'll never look at a 3am wakeup the same way again.
Dr. Ray Peat, the biologist and physiologist whose work on cellular energy has impacted how I think about nutrition and hormones. He spent a lot of time writing about the relationship between blood sugar, stress hormones, and sleep. His research on sleep is one of the best explanations I've come across for why so many people fall asleep fine and then bolt awake two or three hours later, heart pounding, mind suddenly racing.
Here's what's happening: your liver stores glucose as glycogen, and that glycogen is what your brain and body use for fuel overnight while you're fasting. The problem is those stores are limited. For a lot of people, especially those eating lower carbs or just not getting enough easily digestible carbohydrates or if you're highly active throughout the day, those stores run dry somewhere around the 2 to 4 hour mark.
When blood sugar drops, your body doesn't just shrug and wait it out. It sends out a stress signal: cortisol and adrenaline surge to mobilize stored energy and bring blood sugar back up. Mission accomplished, biologically. But those stress hormones also wake you up. Heart racing. Mind suddenly active. That feeling that something is wrong even though nothing is. That's not anxiety, it’s your metabolic alarm system doing exactly what it was designed to do.

Peat's fix was beautifully simple: a small amount of easily digestible carbohydrate before bed, orange juice, raw honey, milk, or a gelatin-based drink, keeps liver glycogen topped off through the night so the stress response never needs to fire. If you wake up at that dreaded 3am timeframe, have a glass of water with some honey or milk beside you and take some sips, it’ll help you fall back to sleep. Plenty of people who try this report sleeping straight through for the first time in years.
What’s really happening: When cortisol and adrenaline are flooding your system overnight, which are improper signals for this time, it signals stress and not recovery. Cortisol is catabolic, meaning it breaks down tissue, suppresses thyroid function, and directly undermines the anabolic repair work that deep sleep is supposed to deliver. All that HGH release, cellular repair, and brain clearance we talked about? It gets blunted. You might go back to sleep, but you don't get back to deep sleep.

Ashwagandha (Withania somnifera)
Ashwagandha is an adaptogen, meaning it helps your body regulate its own stress response rather than just suppressing it. For sleep specifically it's been studied for its effects on cortisol, and a double-blind placebo-controlled trial in Medicine found that ashwagandha root extract significantly improved sleep quality, sleep onset latency, and morning alertness compared to placebo. GABA (Gamma-Aminobutyric Acid)
GABA is your brain's primary inhibitory neurotransmitter. Think of it as the off switch for an overactive nervous system.
