Let's talk about ashwagandha because it's one of those words that has become so ubiquitous in the wellness world, but most people have no idea what it's actually doing or why it matters.
You've seen it on supplement labels, in smoothie powders, in gummies shaped like bears. It's been slapped on products with no explanation of why it's there or whether the dose is meaningful.
Ashwagandha is the most studied adaptogenic herb in the world, with over 60 published clinical trials behind it and a 3,000-year history of use in Ayurvedic medicine. The research on what it does to cortisol, testosterone, thyroid function, sleep, cognitive performance, and recovery is really impressive. But none of that matters if the dose is wrong or if the absorption is poor.
Let me give you the full picture, because when you understand what this plant actually does, you'll understand why it's worth taking seriously.
FIRST: WHAT AN ADAPTOGEN ACTUALLY IS
Adaptogen is another word that gets thrown around without explanation. It sounds like marketing language, but it's a substance that non-specifically increases the body's resistance to stress while normalizing physiological function. That last part is important. Normalizing, not pushing in one direction.
A stimulant gives you more energy regardless of whether you need it. A sedative calms you down regardless of your state. An adaptogen does something more sophisticated: it modulates the stress response system, helping it respond appropriately rather than overreact or underreact. The concept was formalized by Soviet pharmacologist Nikolai Lazarev in 1957 and later developed by Israeli-born researcher Israel Brekhman, both of whom were working on substances that could improve resilience in military and athletic populations without the side effects of stimulants. Ashwagandha is the most studied plant in this category by a significant margin, and its mechanisms of action are now well-characterized at the molecular level.

HANS SELYE, THE STRESS CASCADE, AND WHERE ASHWAGANDHA FITS
We've covered Hans Selye's General Adaptation Syndrome in other Prime Choice articles and it's worth revisiting here because ashwagandha's primary mechanism maps almost perfectly onto what Selye described.
Selye identified three stages of the stress response: alarm (the acute fight-or-flight surge), resistance (the sustained adaptation where cortisol stays chronically elevated), and exhaustion (where the regulatory systems fail and the body can no longer mount an appropriate response). He argued that most modern health problems arise from people living in the resistance and exhaustion stages of chronic stress, where cortisol has been elevated for so long that the entire hormonal and metabolic system is running in a depleted, reactive state.
Ashwagandha's withanolides, the primary active compounds in the root, modulate the hypothalamic-pituitary-adrenal (HPA) axis directly. The HPA axis is the control system for the stress response: the hypothalamus tells the pituitary gland to release ACTH, which tells the adrenal glands to produce cortisol. Ashwagandha appears to reduce the sensitivity of this axis to stressors, essentially turning down the gain on the alarm system so it doesn't fire as hard or as long in response to the same inputs.
In the landmark double-blind placebo-controlled trial published in the Indian Journal of Psychological Medicine, 60 days of ashwagandha supplementation at 300mg twice daily produced a 27.9% reduction in serum cortisol compared to placebo, alongside significant improvements in perceived stress, anxiety, and sleep quality. A 2019 study in Medicine replicated these findings with similar effect sizes. These are not marginal improvements. A nearly 28% reduction in serum cortisol is a meaningful clinical outcome.

THE CORTISOL-TESTOSTERONE-THYROID TRIANGLE
Here's where the ashwagandha story gets particularly interesting for anyone who has been noticing the downstream effects of chronic stress: fatigue, low libido, difficulty building muscle, weight gain in the midsection, and a general sense of aging.
Cortisol and testosterone are in direct competition. They share upstream precursors in the steroid hormone biosynthesis pathway, and chronically elevated cortisol actively suppresses testosterone production through multiple mechanisms: it inhibits the hypothalamic release of GnRH (gonadotropin-releasing hormone), which reduces LH and FSH from the pituitary, which reduces testosterone production in the testes and ovaries. This is not a subtle effect. Research consistently shows that chronic psychological and physiological stress significantly depresses testosterone in both men and women.
A randomized controlled trial published in the Journal of the International Society of Sports Nutrition found that men taking 300mg of ashwagandha twice daily for eight weeks had significantly greater increases in testosterone and muscle strength compared to placebo, alongside greater reductions in exercise-induced muscle damage markers. The mechanism is the cortisol reduction: when cortisol comes down, the hormonal pathway that was being suppressed opens back up.
The thyroid connection is equally significant, and especially relevant for the pro-metabolic community. Research published in the Journal of Alternative and Complementary Medicine found that ashwagandha supplementation significantly improved T3 and T4 levels in subjects with subclinical hypothyroidism, with an 18.6% increase in T4 and a 41.5% increase in T3 over eight weeks. The proposed mechanism involves both reduced cortisol suppression of thyroid function and direct modulation of thyroid stimulating hormone (TSH) signaling. In Ray Peat's framework, where thyroid function is the central driver of metabolic rate and cellular energy, this finding positions ashwagandha as one of the most practically useful daily supplements for anyone navigating the metabolic consequences of chronic stress.
Cortisol and testosterone share the same upstream building blocks. When cortisol stays chronically elevated, testosterone production is one of the first things to get cut. Reduce cortisol, and the hormonal pathway reopens.
DR RAY PEAT'S FRAMEWORK: WHY STRESS HORMONE REDUCTION IS THE MASTER VARIABLE
Ray Peat wrote extensively about cortisol as one of the primary drivers of aging, metabolic decline, and what he called the degenerative diseases. His framework positioned the stress hormone cascade not as an occasional response to acute threats but as the chronic background condition of modern life, quietly suppressing thyroid function, impairing progesterone synthesis, promoting inflammation, and breaking down the cellular energy systems that every aspect of health depends on.
In Peat's view, the single most important thing most people could do for their health is reduce the chronicity of their cortisol output. WHAT ELSE THE RESEARCH SHOWS
Beyond cortisol, testosterone, and thyroid, the clinical trials on ashwagandha have documented a range of additional benefits worth knowing about.
Cognitive function: a double-blind placebo-controlled trial published in the Journal of Dietary Supplements found that ashwagandha supplementation significantly improved immediate and general memory, executive function, sustained attention, and information processing speed compared to placebo. A follow-up study in Evidence-Based Complementary and Alternative Medicine replicated these cognitive benefits in healthy stressed adults, with participants also reporting significant improvements in sleep quality. The proposed mechanisms include reduced neuroinflammation from lower cortisol and direct neuroprotective effects of withanolides.
Male fertility: a randomized trial published in Evidence-Based Complementary and Alternative Medicine found that ashwagandha supplementation significantly improved sperm count, motility, and testosterone levels in men with oligospermia. The cortisol-testosterone mechanism is part of this, but the research also suggests direct effects on the hypothalamic-pituitary-gonadal axis.
Physical performance and recovery: beyond the testosterone and strength findings mentioned earlier, ashwagandha has been shown to improve VO2 max, reduce exercise-induced muscle damage markers (CK and LDH), and accelerate recovery between training sessions. For anyone using fitness as a tool for managing stress and maintaining metabolic health, this makes ashwagandha a genuinely dual-purpose daily supplement.
Sleep quality: multiple trials have documented improvements in sleep onset, sleep quality, and morning alertness with ashwagandha supplementation, independent of its anxiety-reducing effects. This appears to be partly mediated by the cortisol reduction (which allows the evening cortisol drop that sleep onset requires) and partly by direct modulation of GABA-A receptors by triethylene glycol, a compound found in ashwagandha leaves and roots.
